The only really solid commenting system that I know of is Modern Guestbook / Commenting system (ve_guestbook). It does include Captcha and can also be integrated into the News system via TIMTAB. Example can be seen here.

Subject: Page Comment Plugin
I got the page comment plugin (sk_pagecomments) however I would like one that incorporates some image verification to help prevent spam, as well as a way to manage messages without having to use phpmyadmin to enter the db. Does such an extension exist right now?
